Please remember to bring to your appointment hard copies of the following documents:
1. Proof of Birthdate - birth certificate or passport. If you have permanent resident status, we will need to view that documentation as well (permanent resident card).

2. Proof of Address - a recent utility bill (within 6 months), lease, or mortgage agreement. Please note, we cannot accept driver's licence or phone bills as proof of address.

If applicable, please bring other documents such as proof of custody, court order, medical information.
